My research interests are focused on building clinic-community linkage interventions that fill health service gaps and reduce health disparities, particularly related to the cardiovascular disease burden. I want to focus on improving individual and population-level health outcomes by bridging healthcare system resources (care coordination, pharmacist interventions, and self-management education) with community-level approaches (community-based pharmacists, community health workers, lay advisors) and community-based resources. I have experience in conducting systematic reviews, meta-analyses, and pilot quantitative and qualitative studies. My current work is focused on identifying the capacity for “upstream” community-based interventions to create a sustainable integrated clinic-community partnership interventions.
