My area of research is care coordination/transitional care. My research focuses on understanding the complex issues surrounding the provision of care to elders in hospitals, nursing homes, community, and primary care settings. Specifically, I am concerned with processes, outcomes, and cost of care coordination across all settings. In past work, I have developed an ontology describing nurse care coordination that was subsequently used to quantify (dose) care coordination activities in community care. We developed a care coordination documentation system for use in primary care. Additionally, my teams routinely use claims data to identify costs of care and utilization outcomes. I am currently funded through Agency for Health Care Research and Quality to adapt the Reengineered Discharge process to SNFs. Additionally, I am a co-investigator in Centers for Medicare and Medicaid funded Innovations grant, Missouri Quality Initiative that seeks to reduce unnecessary hospitalization of skilled nursing facility residents.
